Towards a Hybrid Home Care Model

There is a need to innovate home care delivery trying to reach out a broader number of citizens. PROCAREFUL project is trying to scale up the organisational care models through technology to prevent cognitive and physical decline and to reduce social isolation among people over 55 in need of care.

The webinar will present findings about the key factors for successful hybrid home care models. An experts’ ranking of selected relevant solutions will be presented to derive a series of recommendations coming from mHealth and eHealth experiences about the digital transformation of care.
